Change Primary Care Physician

The form opens only when you choose the Open form button. The provider determines form availability and displays its submission confirmation.

The form is hosted by HIPAAtizer. Opening it does not submit a request. Follow the instructions and check the confirmation inside the form after you submit.

A submitted request is not confirmation of a physician change, policy cancellation, payment change or appointment. If you submitted but did not see confirmation, contact HUGS before trying again to avoid a duplicate.
